A 75-year-old woman died at the scene after the SUV she was a passenger in left King Georges Road at South Hurstville and crashed about 9.30pm on Tuesday.
The 86-year-old man driving the vehicle was taken to St George Hospital in a critical condition before he died on Wednesday, NSW Police said in a statement.
The crash is being investigated and a report will be prepared for the coroner.
It follows the deaths of eight people on NSW roads over the Easter long weekend, five of them in regional areas.
An 83-year-old woman died in a collision at the intersection of a country highway south of Parkes on Monday afternoon, while a man died after crashing his motorcycle after a police pursuit in Sydney's inner west.
Other drivers were detected drink-driving and travelling at excessive speeds throughout the long weekend.
Ninety people had died on NSW roads to midnight on Tuesday, up from 75 at the same point in 2023.
